In concert with Merck for Mothers’ Safer Childbirth Cities Initiative, a coalition of funders including Community Health Acceleration Partnership, The Nicholson Foundation, and The Burke Foundation, co-supported the Camden Coalition of Healthcare Providers. The grant allowed the Coalition and its partners to develop and strengthen new citywide data infrastructures to improve connection to services and care coordination for pregnant and postpartum women.
